Every effort will be made to keep your child safe during class time and performances. However, accidents can sometimes happen. To ensure first aid can be administered promptly if necessary, please complete the following:
I give full authority to the leaders of CreoArtists to administer first aid should my child become injured. In the case of a serious emergency, I give permission for my child to be taken to an emergency clinic. (We will make every effort to contact the parents before taking this step.)
I agree to absolve the leaders of CreoArtists from any liability in connection with any injury sustained by my child before, after, or during the CreoArtists 2025 season.
I grant permission for my child to be photographed and videotaped during classes and performances. I understand that this material may be used for promotional purposes.
Participants must commit to attending every rehearsal without exception. During the rehearsal and performance season, we ask that dancers refrain from engaging in activities that carry a high risk of injury (e.g., skiing, snowboarding). With a small team, every dancer is essential. We ask that all participants take responsibility for their health and safety as part of their commitment to the program.
